Clarification on
Document Verification Requirements
With Document Verification
events coming up, we wanted to provide clarification on those documents which
parnents/guardians are required to bring.
When /
Where: This verification process will take place on the following days:
- Saturday, January 28th from 8:30-10:30am AND
12:30-2:30pm at Cupertino Middle School in conjunction with the Majors &
Minors try-out events.
- Saturday, February 4th from 8:30-10:30am in
conjunction with the farm level evaluations.
- The Registration Team is looking into possibly
holding a verification event mid-week in the evening. More to come on
this.
Required Documents:
- All parents of new and returning
players must submit a Volunteer Application Form. This form is available
on the SVLL website at these locations (one for each parent volunteer):

- All new players to the
league must have their residency verified. To do this:
- You MUST bring proof of
age for each player, such as an original birth
certificate (copies will not be accepted).
- You MUST bring
documentation which proves residency within our league's boundaries. Residence
shall be established and supported by documents containing the full residence
which includes parent(s) or guardian(s) name, street address, city, state and
zip code information, dated or in force between February 1,
2016 (previous year) and February 1, 2017 (current year).
You MUST bring one document from EACH of the three Groups
outlined below:
- Group 1
- Drivers License (photo ID of
parent(s) or guardian(s) with qualifying residence address)
- School records (home address of
players parents or guardians)
- Vehicle records (i.e., registration,
lease, etc.)
- Employment records
- Insurance documents (with residence
address)
- Group 2
- Welfare/child care records
- Federal records (i.e., Federal Tax,
Social Security, etc.)
- State records
- Local (municipal) records
- Support payment records
- Homeowner or tenant records
- Military records
- Group 3
- Voters Registration
- Utility bills (i.e., - gas, electric,
water/sewer, phone, mobile phone, heating, waste disposal)
- Financial records (loan, credit,
investments, etc.)
- Medical records
- Internet, cable, or satellite
television records
If you do not live within SVLL's boundaries, but
your child attends a school within our boundaries, you may satisfy residency
requirements via the following:
- A school issued report card or performance record
will no longer be accepted to establish school attendance. Either the
school enrollment form or an official/certified
school enrollment record dated prior to October 1, 2016 can be used to
establish school attendance for the 2017 season.
